Ihave an Apple Watch Series 5 and I've been using the always-on-display since I received the watch. I have no image retention or burn in and I've been using the watch over 2 years. I doubt anyone here knows how common people experience this issue but my guess is that it is not too common so suggesting people buy Apple Care is overly cautious.
